If you would like to invite one or more of your extended family members, or relatives to visit you in the U.S. while you are studying at UCSC, or on OPT, and would like to request a family visit letter from ISSS to assist them in their visa applications, please complete the following form. In order for ISSS to accurately prepare your family visit letter, you must fill out the following form in its entirety. If you are inviting more than 3 people, please send an email to Fvisa@ucsc.edu and include all the information required on the form for the additional relatives. Please indicate "Additional Information for Family Visit Letter" in your subject line.

Please note that ISSS can ONLY prepare family visit letters for F-1 or J-1 students sponsored by UCSC. The family visit letter only serves as a supporting document for the appropriate visa applications of your family members, or relatives. For information on visitor visas, please visit the appropriate U.S. embassy website.
